Just FYI I have a small 3 year old or so Arctic Willow that I got as a 4" pot starter plant from Evergreen Gardenworks.
I have wintered it once; looses all leaves then. In spring the leaves all come back healthy, green ,and vigorous. Then as summer proceeds I get the occasional black spot, then the yellowing, then maybe leaf dying. They fall off very easily. This process progresses increasingly into the fall where it maybe affects 20% of the leaves. Seems to be natural(?). I am in the Philly area so same weather as you.
I am told that high humidity (our summers) and too much wetting of the leaves can cause this in willows. So I will try and water without wetting the leaves.
Not really sure what does this. Next year I may try some early treatment with fungicide and see if anything is different.
Overall my small tree (now in a Bonsai pot) seems basically healthy.

Unless you have them in a greenhouse with high humidity, they tend to look like yours does and worse in the summer heat. We grow them and its easily 90-110F here in the summer and they always look like they are dying. Once they drop their leaves, and overwinter, they come back lush and beautiful
